    Photo/Video Consent

    Do you consent to the team taking photo and video of your child throughout the season strictly for the use of promoting the team internally within the Hedingham neighborhood? *

    New Swimmers

    If you have a new swimmer joining the team, please list their name and state what's their current swimming ability.

    Returning Swimmers

    If any of your swimmers have been on the Hedingham Sharks Swim Team previously, please list their names and how many years they have been on the swim team (not including 2024 season)?

    Year Round Swimmers

    If any of your swimmers swim on a year round team, please list their names, what team they swim with, and whether they will practice with the Hedingham Sharks, with their year round team, or a mix of both depending on what days.

    Volunteering

    Are you interested in taking on any season long volunteer positions? Such as photographer (at swim meets), fundraising, finding sponsorships, social media posting, head timer, learning meet management / timing software to be a backup at meets, stroke and turn judge, etc.. If you have any other options / roles that you're interested in that would benefit the team please let us know. We're always looking for help.

    Liability Waiver

    By clicking below, the undersigned does hereby waive and release any and all claims, causes of actions. lawsuits and demands against Hedingham Community Association, its management company, its officers, directors and employees (collectively, "Hedingham") arising out of and related to my child's participation in the Hedingham swim team. In addition to the foregoing, the undersigned irrevocably and unconditionally warrants and agrees to defend, indemnify and hold harmless Hedingham for any claims asserted against Hedingham arising out of or related to my child's participation in Hedingham swim team activities.
